DePelchin is seeking an energetic, compassionate, and care-driven Clinical Case Manager. The ideal candidate must live within Region 5 (or be willing to relocate at your own expense) and be able to drive throughout the region performing a wide range of social service activities as they relate to adoption and post adoption. Region 5 counties include Angelina, Hardin, Houston, Jasper, Jefferson, Nacogdoches, Newton, Orange, Polk, Sabine, San Augustine, San Jacinto, Shelby, Trinity, and Tyler. The Clinical Case Manager will:
  • Carry a caseload and provide case management and clinical support services to client families enrolled in the Post Adoption Contract Services program, to include travel to and from required home visits and participation in advocacy efforts to support adoptive families enrolled in this program.
  • Provide community referrals and resources to help adoptive families address their individual needs as they relate to adoption and the effects of trauma, and to help preserve the adoptive family.
  • Develop and facilitate support groups within the DFPS Region 5 Post Adoption service area.
  • Attend community and team meetings, activities and events that provide outreach opportunities and promote program services.
  • Participate as needed in Buddy Family Network events.

Education

Required

Bachelors or better in Human Services or related field.


Preferred

Masters or better in Human Services or related field.

Experience

Required

Minimum of two years experience with target population (children and families) or similar professional work required.

Must be able to maintain high load of telephone and e-mail communication and handle after-hours on-call when scheduled.

Must be willing to drive throughout the region for home and other field visits.

Must be willing to work outside normal business hours (i.e. evenings/weekends) to support families and attend agency-sponsored events.

Must be proficient in Microsoft Office (Word, Excel, Outlook).

Must have a valid Texas Driver License and have reliable transportation.


Preferred

One (1) year of child welfare experience is highly preferred.

Live within the southern region of Texas highly preferred.
