Description
We are looking for RN’s that have a strong sense of initiative, compassion and drive to be responsible for managing plans of care for patients and families receiving hospice care. This is a rewarding opportunity to meet, work with, stimulate and teach a variety of people with different needs at the end of life.
The Coming Home RN Case Manager works with patients and families in the private home setting to provide top-of-the line hospice care.
Position Requirements
Registered Nurse (RN)-State of Ohio
CPR Certification
Certified Hospice and Palliative Nurse (CHPN) preferred
Active Drivers License
Valid Automobile Insurance
