Education/Experience:
Bachelor's degree in health education, health care administration, nursing or social work. Master's degree preferred. Current RN license required. Experience in maternal-child field required. Managed care related experience preferred. Two years experience in community health or primary care setting required. Proficient user of Microsoft Office's Word and Excel programs.
Description:
Ability to effect change as measured in outcomes; develop systems to monitor, track, report program needs/changes as identified; interpret medical records and test results and provide assessment accordingly. Correspondence among members and primary physicians to evaluate total needs of high-risk cases. Facilitates the delivery of medical and/or behavioral health services provided to the member with providers, practitioners, other departments and the Medical Director. Communicates total member/caregiver needs on high-risk cases by reporting changes to management.
Responsible for maintaining a positive working relationship with member/caregiver, physicians, Home Health, DME providers, Inpatient Outpatient facilities and Ancilary Services, etc. Maintains open channels of communication with all health care team members.
