Apply your professional knowledge and skills to meet the needs of the patient and help achieve individualized outcomes. Assess and evaluate therapeutic/rehabilitative/functional status and confer with the physician in developing the total plan of care. Evaluate home environment. Assess for muscle strength, mobility, gait, ROM and transfer capabilities. Direct physical therapy treatment. Supervise the P.T. Assistant and home health aides as needed. Instruct patients/families/care givers in the use and care of therapeutic appliances.
Qualifications:
Must possess a degree in physical therapy approved by an accredited organization. Licensed to practice as a Physical Therapist in New York State. Two years of experience as a Physical Therapist and community/home health experience is preferred. Must possess and maintain CPR certification and have current NYS driver's license.
Requirements: BS in Physical Therapy and NY license or temporary license if new grad.
