Position Summary:
Responsible for following the Plan of Care established by the Occupational Therapist, coordination of treatments with assigned patients, and a continuous review of treatments provided to the patients and their response. of the job. Performs the skill/tasks as identified in the Plan of Care created by the Registered Occupational Therapist and documents according to agency policy. Assists the Registered Occupational Therapist in notification of supervisory visit dates that are required monthly. Reports concerns, events, patient progress to supervising Registered Occupational Therapist. Utilizes supportive equipment and supplies as needed and sees to cost effective use of resources. Documents communication with physicians and home health staff. Participates in weekly case conferences with interdisciplinary team to communicate Plan of Care, concerns and progress. Attends VCHH meetings, 75% attendance is mandatory. Maintains and expands knowledge base by attending professional educational programs Mentors students and new staff to home health care. Demonstrates concern and support of the mission and philosophy of Via Christi Home Health and behaviors consistent with the Via Christi Standards of Conduct. Education and/or Experience: Associates degree from Accredited COTA program. Current Kansas COTA certificate. 1 year of Occupational Therapy experience within a health care community. Prefer previous home health experience.
