General Summary:
Under the general direction of, and in collaboration with the physician, provides diagnosis and treatment of patients within the scope of practice as defined by all applicable Federal, State and local laws, regulations, protocols, etc. and within the individual nurse practitioner's particular clinical speciality area, licensure and competency level. Scope of practice would NOT include inpatients, high risk prenatal patients, and patients with unstable chronic illnesses and life threatening conditions. Performs acts of medical diagnosis or prescription of medical therapeutic or corrective measures as appropriate.
Duties and Responsibilities:
The following are essential job accountabilities:
1 Obtains/records initial patient histories, identifies problems/needs (within the parameters of Nurse Practitioner functions), and designs comprehensive management plans for patients. Refers to appropriate physician(s) or community service(s) as appropriate.
2 Assesses current health status, real and/or potential health problems and identifies expected outcomes in response to management plans, reviewing data with the physician as necessary.
3 Performs physical exams as appropriate.
4 Requests appropriate diagnostic and screening procedures and evaluates the results.
5 Writes prescriptions for required medication and/or treatment to meet management plan as allowed by State Regulations and protocols.
6 Coordinates continued care for individual patients and provides follow up evaluation of patients' response to management plans.
7 Provides emergency care as indicated and supported by protocols, training and experience.
8 Acts as a resource, educator and consultant to staff. Develops and evaluates standards of care and protocols as necessary. Provides patient and family education, support and follow up as needed.
9 Monitors and ensures accuracy of recording on patient medical records. Maintains all required reports, records, statistics, etc.

10 Demonstrates knowledge of the principles of growth and development over the life span of the assigned patient population. Assesses and interprets patient age specific data and provides appropriate, age specific treatment. Provides direct patient care to assigned patient age group(s).
11 Demonstrates a commitment to patient, visitor and staff by: complying with all applicable safety regulations; learning the impact of medical errors and methodology that will lead to reduction of errors; reporting actual and potential errors, as well as hazardous conditions; identifying opportunities to standardize processes and "error proof" systems that will lead to increased safety; and participating in safety education programs and root cause analyses as required.
12 Maintains established policies and procedures, objectives, quality assessment, safety, environmental and infection control standards.
13 Enhances professional growth and development through participation in educational programs, current literature,inservice meetings, and workshops.
14 Provides outstanding service to all customers; fosters teamwork; and practices fiscal responsibility through improvement and innovation.
The following are nonessential job accountabilities for ADA purposes:
15 Attends meetings as required. Participates on committees within the System and/or the community as requested.
Performs other related duties as identified.
Qualifications:
Note: Other combinations of formal education, training and experience may be considered.
Minimum Education Master's Degree with national certification
Minimum Experience 1 - 2 years
Req Cert/Registration Registration and current license as CRNP in PA; appropriate clinical specialty certification; CPR certification
Req Course(s)/Training Completion of an accredited program for nurse practitioners
