General Summary:
Under the general supervision of the Supervisor-Case Management Social Work, plans, organizes and implements social work for patients and families. Works in a team model (Case Management Nurse and Social Worker teams are assigned to patient care areas) to address patient discharge needs.
Duties and Responsibilities:
The following are essential job accountabilities:
1 Assesses patients/families in need of psycho-social support throughout the patient’s hospitalization and assists patients who require post acute care coordination or community resources referrals. Advocates for patient self-determination and appropriate level of care.
2 Identifies discharge plan by making arrangements for psycho-social, psychological and practical assistance across the continuum of care. Actively engages the patient and family in the assessment, problem solving and referral process including transfer and discharge plans.
3 Notifies patient/clinical team of potential referrals, documents on the medical record in a timely fashion: psycho-social assessments, identified plan, barriers, changes and progress toward plan completion. Notifies final outcome and patient satisfaction.
4 Interacts with the interdisciplinary team in the development of the discharge plan addressing the patient’s/family’s psycho-social support.
5 Coordinates family meetings. Assists patients and families in navigating the healthcare system.
6 Performs a variety of functions to address patient’s psycho-social needs with referrals for counseling, community services, protective services, and financial assistance referrals ( MA application).

7 Demonstrates knowledge of the principles of growth and development over the life span of the assigned patient population. Assesses and interprets patient age specific data and provides appropriate, age specific treatment. Provides direct patient care to assigned patient age group(s).
8 Demonstrates a commitment to patient, visitor and staff by: complying with all applicable safety regulations; learning the impact of medical errors and methodology that will lead to reduction of errors; reporting actual and potential errors, as well as hazardous conditions; identifying opportunities to standardize processes and "error proof" systems that will lead to increased safety; and participating in safety education programs and root cause analyses as required.
9 Maintains established policies and procedures, objectives, quality assessment, safety, environmental and infection control standards.
10 Maintains professional growth and development through seminars, workshops, and professional affiliations to keep abreast of latest trends in field of expertise.
11 Provides outstanding service to all customers; fosters teamwork; and practices fiscal responsibility through improvement and innovation.
The following are nonessential job accountabilities for ADA purposes:
12 Prepares and presents utilization data analysis as required.
13 Develops and initiates educational programs regarding utilization management principles.
14 Attends meetings as required, and participates on committees as requested.
Performs other related duties as identified.
Qualifications:

Minimum Education Bachelor's Degree
Minimum Experience 6 - 12 months
Pref Cert/Registration Social work license in PA
Req Course(s)/Training Clearance per Child Protective Services law (11 P.S. 2201) amended by PA Act 33 of 1985
